Supports Coordinator

Role of the Supports Coordinator is to locate, coordinate, and monitor services for individuals.

Insures implementation of all provisions of the Individual Service Plan. This person will be responsible for all aspects which will provide individuals with intellectual disabilities and his/her family with needed services. Liaison with other service providers is essential.
Responsible for completing social histories, developmental assessments, and assessments of adaptive behavior.
To develop annual Individual Service Plans on all individuals with intellectual disabilities assigned to caseload.
To refer individuals with intellectual disabilities to appropriate services as determined by the Multi-Disciplinary Team.
Assist families in applying for funding streams necessary to access appropriate services.
Responsible for face to face contact with client and/or attend collateral staffing to monitor client’s progress, in compliance with the Office of Developmental Programs regulations.
Responsible for documenting all activities as defined by all governmental regulations through HCSIS system.
Schedules and coordinates the implementation of the Individual Service Plan as well as pre-planning activities in accordance with Office of Developmental Programs policy. Serves as point   of reference for clients, families and providers in regards to Developmental Services regulations as they affect service delivery.

Completes all required service notes, and HCSIS specific information on a daily basis.
Develops and maintains a positive and professional working relationship with peers, providers, families, and consumers.
Actively monitors the quality of services provided to individuals with intellectual disabilities.
Coordinate supports and services for individual receiving services.
Maintains written records and reports as needed to support Quality Management.
Adhere to policies and requirements as stated in the Developmental Programs Bulletin effective May 26, 2010, number 00-10-06.
This person may be assigned other related duties as requested or required by the Executive Director and/or his designee.

Requirements

Bachelor’s Degree in special education, education, social work, or related field.

o Master’s degree in special education, education, social work, or related field.

o Experience working with individuals with intellectual disabilities necessary.

o Good interpersonal skills, excellent communication skills, and good documentation skills.

o Knowledge of the MH/DS system.

o Knowledge of community resources.

o Must be proficient in the use of Computer systems and the Internet.

o Must have the ability to effectively communicate both verbally and in writing.

o Must be organized and able to maintain all required documentation and productivity requirements.

o Must be willing to work flexible hours.

o Must have Act 33/34, FBI Clearances.
